Question: Does this program send large numbers of emails (5000 recipients) through a local server?

I need to send emails to participants in a research study from my university email address but the university will not allow me to do this unless it goes through the university server. So I can't use Mailchimp for example.

Answer: Yes, however, the number of emails you can send doesn't depend on MaxBulk Mailer at all. MaxBulk Mailer has no real limit. It actually depends on the mail server (SMTP server) you are using. If your mail server allows you to send to 5'000 recipients at once, just do it.
