I've installed MLM, but URL doesn't work

I've followed all of the instructions carefully to install my MLM found here:

The LM.php appears in the path I requested, and I've confirmed this by logging into my FTP using a web browser (see attachments).

And yet, when I try to visit and log into my list manager using the URL, I get an error message saying the page was not found.

Can someone please help me figure out what I'm doing wrong?

I tried attaching screenshots but the files are too big, and if I shrink them any further they'll be illegible.

Are you really sure the file is located in the place you think it is? With the right privileges? You could also contact your server administrator and see if there is some kind of forward in place.
