The username and password you are referring to are the MLM username and password. For accessing MLM both from the web and MaxBulk Mailer. It is explained both in the MLM manual and in the video.
For database access, you need the R/W one. To create tables and modify them.
In /lm/lm.php?cmd=init&pwd=password you have to use MLM password you choose previously.
If the URL does nothing you should look at the server error log to see the reason.
