How to Place Company Logo in Online Confirmation Web Pages

Hi Guys,
I am trying to personalise the complete signup and unsubscribe experience.
It is particularly difficult with the online confirmation web pages as they appear to be drawn by multiple php scripts and php pages.
Does anyone know where I can insert the relevant code to place a banner image above these pages and also the standard viewport html command for mobile devices?

Look at the 'lm/templates/pageheader.html' and 'lm/templates/pagefooter.html' pages. They are used to display the page header and footer respectively.
