subscribe page for my website

What do I need to change on the template or the html page to put up a page to accept new users that want to join my mailing list?

I don't know exactly how to change the html or if I have to add my own code or what?

I have tried to modify the subscribe_single_selection.php page but with no luck.

Everything else works for the mlm. I'm at the point where all I really need is for a form to acquire the email of those interested to automatically be added to my list

All you need to change is the part where you select the list. You have to replace the default list name(s) with your(s). In 'subscribe_single_selection.php' it is a pull-down menu.
