Sending Forms

I need to send professional looking forms to my clients in MaxBulk Mailer. For example sending them a form checking there current address and contact details. Does anyone know how I can do this. Many thanks

You have to design your form with an HTML editor like Dreamweaver. The form has to call a script in your site. I guess it should work but I have never tried.
