I got a Wordpress plugin developer to create a Widget to interface with MLM. It has to be installed, activated, placed and set as any plugin:
1.- Unzip the file
2.- Upload the 'mlm_widget' folder to 'wp-content/plugins'
3.- Go to Wordpress -> Plugins and activate 'MLM'
4.- Go to 'Appearance -> Widgets' and add the Widget
5.- Click on the disclosure triangle on the left of 'List Manager'
6.- Enter the settings
The fields list support things like:
lastname[required=no]
or lastname[title=Apellido&required=no]
