Cannot upload to database

I'm trying to upload a local list to LM.

I have 3 tables:

tbl_mlm_sub
tbl_mlm_sub_clk
tbl_mlm_sub_loc

2 tables are increasing with entries as people unsubscribe and click-through.

But... I cannot upload a list to LM

I have no ideas how to troubleshoot this issue.
