I used your sample url to send the MaxProg image and that worked fine. When I use my own domain info, the image link appears as a broken link; however, the date file gets incremented. What could I be doing wrong here?

Are you getting broken link messages that say permission denied. If so you have to set the permissions on your server.

The shared hosting requires that I use the php4 file extension.

As I mentioned, the data file gets incremented, so I don't think it is a permissions problem, but I could be wrong. I put the click thru files at the top level of the site and set the folder permissions to 666 and also tried 755.

The script is not ran actually. In the script the is a forward. It is simply ignored, I guess you have to ask your web hosting service to activate some PHP setting.
